2020 Pursuit of Excellence Awards Nomination Form

Thanks for submitting a nomination for the 2020 Faculty of Nursing Pursuit of Excellence Awards!

To qualify a nomination, please ensure you have included:

• A completed nomination form with two signed letters of reference (one may be from the nominator). Letters should not be longer than two pages. Please do not include more than two letters of reference: if more are included, the committee will blindly choose two and eliminate the rest.  Letters MUST BE SIGNED so please sign, scan and submit the letters. If the letters are unsigned, the nomination will be rejected. 
• If you are nominating in the undergraduate or graduate student category, one of the letters of reference must be from a faculty member or professional colleague.

If you are submitting for the Excellence in Research Award, you must also include:

• CV of nominee (no more than 2 pages). CV is only required for the Excellence in Research Award category. 

To strengthen the nomination and ensure the committee is fully aware of the nominee’s suitability for the award, please refer directly to the criteria of the award in your letter(s) of support.

Because the Faculty of Nursing Pursuit of Excellence Awards are a renaming of the Covenant Health Faculty of Nursing Awards, no previous recipient may be nominated. Visit our website nursing.ucalgary.ca/excellenceawards for reference to past award winners.

Only completed and qualifying nominations will be accepted. Nominations close February 28, 2020.
